1. Arrive 15 minutes before your appointment for paperwork

2. We will need a copy of your photo ID

3. Bring a copy of your insurance card

1. If you have a new insurance card, bring this in and let us know prior to your appointment

2. We are requiring a yearly consent form to be filled out
